model release

Please review the following statements to the best of your ability. Fill in all form fields and submit your model release form. If the form does not cooperate, I will have a hard copy at the studio for you to sign. Thank you!

• I hereby authorize Willow & Ferns Photography to publish photographs taken of me, and my name, and likeness, for the purpose of display, portfolio, advertising, website and any publication to promote his/her/its studio or name.

• I hereby release and hold harmless Willow & Ferns Photography from any reasonable expectation of privacy or confidentiality for myself and/or any minor child and/or children listing below associated with the images specified above.

• I further acknowledge that participation is voluntary and that neither I and/or the minor or children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in Willow & Ferns Photography marketing materials or other publications.

• I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release Willow & Ferns Photography, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation and/or the minor children listed below.

• The negatives and or digital images (hereinafter collectively the “images”) created by the Photographer and/or subcontractor(s) remain the property of the Willow & Ferns Photography.
